Healthy Yolo brings together community members, non-profits, government agencies, and healthcare to collectively build a Yolo County where everyone has what they need to thrive. Healthy Yolo is coordinated (backboned) by staff from the Public Health Branch of Yolo County HHSA.

    • Three Community Health Assessments (CHAs) and two Community Health Improvement Plans (CHIPs)
    • A resident Community Advisory Board for CHIP development that included eight residents from throughout the County.
    • A countywide farmworker assessment

      Requirement: These meetings are the "in-person" requirement for accreditation.
      Provided: Continuing Education units are provided for each meeting.
