Permit Application Form: While the well owner or the C-57 well driller can sign the permit application (link above), a C-57 licensed well driller must be identified for the work. Most of the time the well driller completes the application form. A non-inclusive list for the well drillers is linked above.

Setback requirements: See related link above. The setback to the leach field does not apply to the abandoned one. However, it does apply to the 100% replacement area (an area of land dedicated for replacement of an entire septic system upon its failure). For properties connected to the City sewer system which have no septic system on site serving as a back-up, this setback to the 100% replacement will not apply. Note: all set-back requirements, such as wells to septic tank/leach field (replacement area included) and sewer line will include those site features located on one's own parcel as well as neighbors' parcels.

Backflow prevention requirement: To protect the approved drinking water source, a backflow preventer shall be installed at each drinking water service line approximately 5 feet into the property line per the following Title 17 of California Health and Safety Code and City of Davis Municipal Code.

(1) Premises where there is an unapproved auxiliary water supply which is interconnected with the public water system: Minimum type of backflow prevention is an Air-Gap (AG). A Reduced Pressure Principle backflow preventer (RP) may be provided in lieu of an AG if approved by the health agency and water supplier

(2) Premises where there is an unapproved auxiliary water supply and there are no interconnections with the public water system: Minimum type of backflow prevention is RP.

In addition, at the minimum, RP device must be tested by a certified backflow tester and the result submitted to the water supplier for review and filing on the annual basis. A list of certified backflow testers can be found at this link maintained by the City of Davis.
