Referral and Criteria

To be eligible for the Yolo County Nurse Home Visiting Program, participants must:

• Live in Yolo County
• Be pregnant or parenting a child under the age 2 1/2

To be eligible for the Yolo County Adolescent Parenting Program, participants must:

• Live in Yolo County
• Be pregnant or parenting adolescent under the age of 19 with a child under the age of 2 1/2

In addition to the above criteria, participant may meet the following criteria:

• Birthing parent or infant with a high risk or complicating medical condition such as gestational diabetes or recently discharged from the NICU
• History of previous poor birth outcome, for example: prematurity, fetal or neonatal demise
• Family history of child neglect or abuse (Birthing parent or baby)
• Birthing parent mental health and/or alcohol and drug problems
