External Funding Request Application - This application is for community-based organizations interested in applying for CCP funding to provide programs and services that align with the CCP's mission and advance its Strategic Plan.  Proposals that additionally seek to prevent or remediate the impacts of systemic racism and/or disparate outcomes will be given highest consideration.  The application period for the funding distribution period July 1, 2024 to June 30, 2026 was open February 8-29, 2024 and has now been closed.

Internal Funding Request Application - This application is for funding requests from CCP departments and agencies.  Internal Funding Requests are due by March 15, 2024.
